다음 양식을 다운로드 해서 제출하기 바랍니다.

• Application form 신청서
• Summary of study 연구요약서
• Form for the external data collection: if data collection procedure will be made on outside of Woosong university
  외부 장소 자료수집 요약서 : 우송대학교 외의 장소에서 데이터 수집 시 제출
• Continuation of study : if investigators intend to extend the study more than one year
  지속연구 : 연구자가 1년 이상 연구를 연장하길 원할 경우
• Termination of study : if the study will be closed before the date of the IRB approval
  -Submission deadline :
  1) Within the approved period.
  2) Within 3 months after the approved period is closed.
  
  연구 종료 : IRB 유효기간 전에 연구가 종료된 경우 또는 승인된 연구기간이 끝난 경우
  - 제출 기한 :
  1) 승인된 연구기간 내
  2) 승인된 연구기간 종료 후 3개월 이내

Upon the first submission: forms 1,2,3, study protocol explaining study procedures in detail (free form)
and study proposal (free form) should be submitted

첫 번째 심사 시 첨부서류 : 신청서, 연구요약서, 외부 장소 자료수집 요약서(해당 경우), 연구절차가 자세히 기술된 프로토콜과 연구계획서(자유양식)를 제출해야 합니다.

Fees

• 100,000krw per research upon submission 100,000원 / 1건 심사
• bank account for the fee : 44020014550 sc제일은행 이수복
• The IRB will proceed the further process after confirming the fee
  요금을 확인한 후 심사절차가 이뤄질 것입니다.
